Registration for new students for 24/25 will being in June.
Once you are registered in FOCUS, the Registrar will contact you to complete the registration process.
The following documents are required to enroll a new student:
- Student's birth certificate
- Proof of residency - (examples of acceptable proof of residency) utility bill, water bill or signed lease agreement or mortgage. We will not accept trash, cell phone or cable bills
- Immunizations - Florida Certification of Immunization, Form DH-680. This form must be acquired from a health care provider. (You will have 30 days from student's first day of school to turn the 680 form in) . Students entering 7th or 8th grade must provide proof of TDAP vaccine to enroll.
- Physical - Florida School Entry Health Exam, Form DH-3040. . A student entering a Florida school for the first time must have a physical examination by a physician within the first 30 days of school. Documentation from the physician must be submitted to the school within 30 days of enrollment.**
- School Records (we will accept an unofficial transcript or student's last report card and current grades until we can get records from previous school)
- Documentation for IEP, 504, or Gifted placement (if applicable)
- Custody Paperwork (if applicable)
- Notarized Co-Living Form (if applicable)
- Parent/Guardian’s driver’s license
